Your AmeriHealth Caritas New Hampshire ID Card
Now that you're a member, you will get an ID card.
Your AmeriHealth Caritas New Hampshire ID card is mailed to you within seven days calendar days of your effective enrollment date. We use the mailing address on file at the New Hampshire Department of Health and Human Services. Your card will have your primary care provider’s (PCP’s) name and phone number on it. It will also have your Medicaid identification number and information on how you can contact us if you have any questions.
If anything is wrong on your AmeriHealth Caritas New Hampshire ID card, call us right away.
If you don’t receive your card, or if you lose your card, we can help. Call Member Services at 1-833-704-1177 (TTY 1-855-534-6730). You can also request a new ID card through our online member portal.
If you need to use your medical, pharmacy, mental health or substance use benefits before you get your AmeriHealth Caritas New Hampshire ID card, please call Member Services at 1-833-704-1177 (TTY 1-855-534-6730). We will give you your member ID number. Write down your member ID number and take it with you when you go to your health care provider or pharmacy. Your health care provider should call AmeriHealth Caritas New Hampshire to check your eligibility.
You can also log in to our member portal to view or print a copy of your member ID card information. Your electronic member ID card is available within 24 hours of enrollment in our plan.
Every member gets his or her own ID card. Do not let anyone else use your AmeriHealth Caritas New Hampshire ID card.
Carry your ID card with you at all times.
Why you need your ID cards
While you are a member of the plan, you must use your AmeriHealth Caritas New Hampshire membership card whenever you get covered services or prescription drugs. However, even if you don’t have your plan membership card, a provider should never deny care to you. If a provider refuses to treat you, call Member Services, 24 hours a day, seven days a week, at 1-833-704-1177 (TTY 1-855-534-6730). We will verify your eligibility for the provider.
As long as you are a member of the plan, you must use your AmeriHealth Caritas New Hampshire membership card to get covered services. Keep your New Hampshire Medicaid card too. Present both your plan membership card and New Hampshire Medicaid card whenever you get services.
You might need to show your medical insurance cards:
- At every health care provider visit.
- At a pharmacy.
- If you visit a behavioral health provider.
- If you need non-emergency transportation.
To get service, you may need to show your:
- AmeriHealth Caritas New Hampshire ID card.
- Medicaid ID card.
- Other health insurance cards you may have.
