From health tips to local resources, the AmeriHealth Caritas New Hampshire Wellness and Opportunity Center offers something for everyone.

By walk-in or appointment

Walk-in:

  • Use computers for job searching, resume writing, or completion of your Health Risk Assessment.
  • Learn about plan benefits.
  • Get referrals to community services.

Appointment (scheduled by your Care Manager):

  • Meet with your Care Manager in person.
  • Discuss management of chronic conditions such as asthma, diabetes, or chronic pain.
  • Get in-person assistance to complete your Health Risk Assessment.

The Wellness and Opportunity Center holds a variety of special events. These events bring neighbors together to learn more about healthy living.
