Important First Steps for Young Adults

Put the following in a safe place to access as needed:

ID Card — bring your AmeriHealth Caritas New Hampshire ID card with you to all of your medical appointments and to the pharmacy. Bring your State of New Hampshire Medicaid card also.

Welcome Letter — Your Welcome Letter will tell you important things you need to know, such as the types of questions Member Services can answer for you, as well as where to find your Member Handbook and Reference Guide on our website.

Member Handbook (PDF) provides more in-depth information about your benefits, contact information, how to appeal a decision you don’t agree with, and much more.

If you would like a copy of the Member Handbook or Reference Guide mailed to you at no cost, please call Member Services at 1-833-704-1177 (TTY 1-855-534-6730).

With the AHC Mobile app, you have ready access to your AmeriHealth Caritas New Hampshire ID Card as well as your Member Handbook.

Register using the member ID number that is on your member ID card.

Choose the email address you want to use as your user ID and create a password.

Look at the letter that came with your AmeriHealth Caritas New Hampshire ID Card.

Want to choose a different doctor? You can change your PCP through your member portal or by calling Member Services.

If your contact information changes, you need to update both:

• New Hampshire Department of Health and Human Services by visiting NHEasy
• AmeriHealth Caritas New Hampshire: Call Member Services at 1-833-704-1177.  

If  you attend school in another state, check with your school to see what health care benefits are available to you through the school. Some schools offer as-needed health care or Student Health Services included with your tuition.

You should schedule routine visits for when you know you will be in-state.

If you are out of state and you need care that can’t wait, you may get care at an urgent care center or emergency department. Give them your State of New Hampshire Medicaid card and AmeriHealth Caritas New Hampshire ID card. Ask them to bill us directly. You are not required to pay for these services at the time of service.

Some providers in our network offer telehealth services. You use our online Provider Search and select “Places by Type” and search for “TeleCare.” This means you can visit an in-network provider through video without being at the same location. This only works for certain types of services such as mental health counseling.

Your family can choose to be on one health plan or split up between plans. Although, each person can only be a member of one health plan.

Yes. Members may be eligible for both Medicaid and Medicare, depending on income level. The state determines your eligibility.

There is no member premium for your New Hampshire Medicaid managed care plan. New Hampshire Medicaid is a federal and state funded health program. A premium is the periodic payment paid to an insurance company or a health care plan by a member or other party to provide health care coverage.

Once you are enrolled with AmeriHealth Caritas New Hampshire, you will receive a Welcome Kit. The information in this kit tells you about our plan, benefits, and services. The kit will include the Member Handbook and member ID card. It will also have information on how to change your primary care provider. You can also access this information online through our Next Steps for New Members section.

You can use our online provider search tool to find a provider already in our network. 

You can also call Member Services at 1-833-704-1177 to have a Provider Directory mailed to you or for help finding which providers are in the AmeriHealth Caritas New Hampshire network. 

If you are joining AmeriHealth Caritas New Hampshire from another Medicaid Managed Care Organization (MCO) then your standard medical benefits (See section 4.2 of your Member Handbook) will not change. Each MCO offers extra benefits such as rewards for completing healthy activities. These extra benefits vary from MCO to MCO. Routine care should be received by a doctor that is part of our AmeriHealth Caritas New Hampshire network of providers. You can search for in-network doctors.

New Hampshire borders multiple other states, and there may be some providers that are part of AmeriHealth Caritas New Hampshire’s network who are located in a neighboring state. Use our provider search tool  or call Member Services to check if the provider you want to see is part of our network.

The term used to describe the plan’s practice of assisting members with getting needed services and community supports. Care coordinators make sure participants in the member’s health care team have information about all services and supports provided to the member, including which services are provided by each team member or provider. For more information, refer to Section 5.2 (Care coordination support) in your Member Handbook (PDF).

An amount you may be required to pay as your share of the cost for a medical service or supply, including a doctor’s visit, hospital outpatient visit, or a prescription drug. Under our plan, you may have a prescription drug copayment.

Refer to the Benefits Chart in Chapter 4 of your Member Handbook (PDF) for a list of covered services.

A “medical emergency” is when you, or any other reasonable person with an average knowledge of health and medicine, believe that you have medical symptoms that require immediate medical attention to prevent loss of life, loss of a limb, or loss of function of a body organ or part. The medical symptoms may be an illness, injury, severe pain, or a medical condition that is quickly getting worse. Or in the case of a pregnant person in active labor (meaning labor at a time when there is not enough time to safely transfer you to another hospital before delivery, or the transfer may pose a threat to your health or safety or to that of your unborn child).

A prescription drug that has the same active-ingredient formula as a brand-name drug. Generic drugs usually cost less than brand-name drugs. The U.S. Food and Drug Administration (FDA) rates these drugs to be as safe and effective as brand-name drugs.

Services, supplies, or prescription drugs needed for the prevention, diagnosis, or treatment of a medical condition and meet accepted standards of medical practice. For more information about medically necessary services, refer to Section 6.1 (Medically necessary services) of your Member Handbook (PDF).

Doctors, pharmacies, and other health care professionals, medical groups, hospitals, durable medical equipment suppliers, and other health care facilities that have an agreement with the plan to accept our payment and your cost-sharing amount, if any, as payment in full. We have arranged for these providers to deliver covered services to members in our plan.

These services are covered by the plan if you are unable to pay for the cost of transportation to provider offices and facilities. The plan covers non-emergency medical transportation to medically necessary New Hampshire Medicaid covered services listed in the Benefits Chart in Chapter 4 (Transportation services — Non-emergency medical transportation [NEMT]) of your Member Handbook (PDF).

A premium is the periodic payment paid to an insurance company or a health care plan by a member or other party to provide health care coverage. There is no member premium for your AmeriHealth Caritas New Hampshire Medicaid managed care plan.

The network doctor or other provider you see first for most health problems. The PCP will make sure you get the care you need to keep you healthy. They may also talk with other doctors and providers about your care. Refer to Section 3.1 (Your Primary Care Provider (PCP) provides and oversees your medical care) of your Member Handbook (PDF).

Urgent care is urgently needed services or after-hours care provided to treat a non-emergency, unforeseen medical illness, injury, or condition that requires immediate medical care to prevent a worsening of health due to symptoms that a reasonable person would believe are not an emergency but do require medical attention. Urgently needed services may be furnished by network providers or by out-of-network providers when network providers are temporarily unavailable or inaccessible.
